FHS Library Media Center Equipment Check Out Policy

The FHS Library Media Center has equipment that can be checked out by students overnight. This equipment includes the following items and their estimated price of replacement:

Laptop Computer $1600.00
Digital Camera $500.00
Zip Drive $200.00
Zip Disk $15.00
Computer Case $45.00
Digital Camcorder $1200.00
Mobile Disk Storage $150.00

The purpose of this policy is to provide ALL students the opportunity to use equipment both during and after school hours. Equipment check out is a privilege -- If students do not return the equipment on time, another student may be without a laptop for class.

A signed Equipment form (available from the Library Media Center and through this web page) must be on file in the Media Center before any student will be allowed to check out equipment. The student and their parent/guardian must sign the form showing their responsibility. This form must be completed each school year. Each time a student checks out equipment a teacher must sign a note stating the student needs the equipment for a classroom assignment.

The Student Equipment Requested Form should be completed and returned by the student to the FHS Library Media Center where it will be kept on file for the current school year.

Equipment is checked out after school for overnight use and must be returned to the Media Center before school begins the next school day. If equipment is not returned on time the following actions will be taken:

1. Media Center personnel will determine if the student is in attendance. If the student is in the school building, the Media Specialist will go to the first period classroom of the student and talk with the student. The student will either be sent home to get the equipment or will be allowed to contact a parent, etc. to return the equipment before the first period class is over. If equipment is returned before the first class period is over, the student will receive a warning that all future equipment checked out needs to be returned on time or they will lose the equipment checkout privileges for one week.

2. If a student is not in attendance, Media Center personnel will attempt to contact the student or parents at a contact number the student provided to the school at registration time. If the equipment is returned before the first class period is over, the student will receive a warning that all future equipment checked out will need to be returned on time or they will lose the equipment checkout privileges for one week.

3. If equipment is returned the day it is due, but AFTER the ending of first period, the student will lose equipment checkout privileges for one week.

4. If a student does not return equipment on time a SECOND TIME, the student will lose equipment checkout privileges for the rest of the semester.

5. If equipment is not returned the day it is due, the student will lose equipment checkout privileges for the rest of the semester.
